Discrimination Complaint Form


Northeastern University values diversity, inclusion, and an environment free from discriminatory conduct. The University is committed to providing a safe, respectful educational and work environment that is free of harassment and intimidation.

The University's policies that prohibit discrimination offer an effective and efficient process for individuals and/or groups to report allegations of discrimination. Community members who have experienced or witnessed discrimination may use this online form to make the University aware of the incident.

If this is an EMERGENCY situation, CALL 911 from off-campus or contact the Northeastern University Police Department at 617-373-3333. Do not use this form to report events that present an immediate threat to health or safety.

When reporting allegations of discrimination through this form, please also note the following:
• Your confidentiality and trust is very important to us. We will make every effort to respect your privacy. Please be aware, however, in certain circumstances involving safety or criminal action, we may not be able to guarantee complete anonymity. Regardless of the situations, matters are always handled discretely and in conformity with University privacy policies, and information about individuals or situations will only be shared with individuals with a legitimate need to know. The University also has strong anti-retaliation policies that protect individuals who report incidents or participate in an investigation.
• Anyone reporting an incident through this portal should provide either a phone number or an e-mail address so that we may contact you if there is a need for clarification or if additional questions or concerns arise. Also, if you want to receive a copy of your report, you will need to enter an email address in the top section.
• Students may also contact on-call campus medical and psychological professionals at UHCS by calling 617-373-2772 or through Find@Northeastern which offers 24/7 mental health support via phone at 877.233.9477 (in the U.S.) or +1.781.457.7777 (outside the U.S.).
