It came to pass in the city of Lublin on the 28th of July/10th of August 1914 at 9:00 AM. There appeared [before us] the Jew Herszek Rozenbojm, carpenter, age 30, resident of the settlement of the settlement of Komarówka [Podlaska], Żelizna Township, Radzyń County, in the presence of the witnesses Icek Majer Horowicz, private tutor, age 41, and Berek Tuchman, homeowner, age 51, residents of the city of Lublin, and showed us a boy, stating that he had been born in the city of Lublin in House No. 455 on Rosyjska Street on the 17th of February/2nd of March 1911 at 2:00 PM to him and to the unmarried woman Dwojra Szwarcman, age 28, resident of the city of Lublin. This boy was given at his circumcision the name Szyja. The belated reporting of this event was not justified in any way. This document was read out to those present and duly signed by them – except for the petitioner [Herszek] who is illiterate – and by us.
