Proof of purchase must be submitted online at CZ-USA-Rebates.com. Acceptable proof of purchase must include a photograph of the original product UPC, and either a copy of a dated cash register receipt or a dated itemized sales invoice. This offer may not be combined with any other CZ-USA offer. Please allow 6 to 8 weeks from submission receipt for delivery of Mastercard® Prepaid Card. Limit of one rebate per Serial # of a new qualifying product purchased. In order to receive your Mastercard® Prepaid Card you must purchase a new qualifying product between 07/01/2023 - 12/31/2023.
Employees of CZ-USA and its affiliates, retailers, clubs, groups, distributors, and resellers are not eligible for this promotion and any entries by such entities are void and will not be honored, acknowledged, or returned.
