Wellness on Wheels Needs Survey

Berks Community Health Center is providing primary and preventative care throughout Berks County in their Wellness on Wheels mobile medical unit. To identify the needs in the various regions in Berks County, we are asking you to complete this survey so we can improve access to care where it’s needed. The survey is only 4 questions.
