Report Hate and Bias Form

This form is for individuals to report UCD concerns of hate, bias, harassment, and discrimination based on protected categories.

For concerns that are not connected to a protected category, please see HDAPP's complaint type web page for information on where to report other concerns. Specifically for sexual harassment and sexual violence concerns, you are welcome to report concerns using this form and/or through the form specifically designed for those concerns.

Reporting Party Information
By not providing your name and contact information, we will not be able to provide a status update of the case. Responsible employees are required to disclose their name. When deciding whether to report anonymously (i.e., not submitting your name/contact information), we encourage you to read our webpage (hdapp.ucdavis.edu/anonymous-reporting) to understand the possible implications with anonymous reporting.
Complainant's Information (if known)
The complainant is the person who experienced the alleged conduct. By providing this information, HDAPP will generally reach out to the complainant to (1) share resources, (2) offer the opportunity for them to participate in the process, and (3) provide them updates on the status of the case. If you, the reporting party, are also the complainant, you do not need to fill out this portion too.
If you are the complainant, please share what accommodations (i.e., language interpreter, closed captioning, more time, etc.) you would like to request to assist you in participating in our processes. It is important to note that not all accommodations can be approved.
Respondent Information (if known)
The respondent is the person who is accused of the alleged conduct. If a respondent's name is not reported, we may be limited in our response. If respondent's name is unknown, please enter "Unknown" in the name field.

Here is an example of how you can explain the concerns and how it is connected to a protected category: You were not given a project because of your age. You came to this conclusion because your supervisor continues to make negative comments about how old you are and treats younger employees differently (e.g., they are always offered to do projects first).
