About NJHIN

The New Jersey Health Information Network (NJHIN) is a state-wide information system designed to facilitate the secure electronic exchange of patient health information among health care providers and health care systems.

By connecting to NJHIN, health care practices and facilities gain access to a trusted network of data sources, enabling efficient communication, streamlined administrative processes, and improved patient care.

New Jersey Department of Health has engaged NJII as our partner to provide full technical support for NJHIN onboarding and integration.

A consistent and reliable way to match patients across organizations, applications, and services statewide regardless of electronic medical records system to improve patient safety and data integrity and enable data exchange.

Admission, Discharge and Transfer Alerts


Providers receive alerts about their patients’ hospital events enabling follow-up to improve patient outcomes and reduce hospital readmissions.

Active Care Relationship Service (ACRS)


The ACRS is a tool through which providers can identify patients under their care and choose how they wish to receive information about their patients.

Continuity of Care Documents (CCDs)


CCDs are a summary of the clinical care received by a patient at a provider. The exchange of CCDs enables providers to receive information needed to care for their patients.
