Deaf and Hard of Hearing - Custom Request

Terms and Conditions

This form is specific to requesting ASL Interpreting, Real-Time Captioning, and other disability-related accommodations for meetings and events outside of class. While BMAC will endeavor to fulfill all received requests, last minute requests may not be fulfilled. For large events, please provide 2+ weeks notice.

Please note:
- For virtual events, please specify within the "Notes" section the host contact and Zoom link.
- Insert your email address to begin making a disability-related request for a meeting or event.

Students/Student Organizations will not incur charges and do not need to provide any Index Number for Bill Back.

Attention Faculty/Staff/Campus Partners: Sponsoring departments are responsible for the costs associated with disability-related access as these are part of the overall expense of hosting a University event. Service Memo will be sent following the event for billing. Departmental cancellations made with less than 24 hours notice may incur charges.
