- Documentation of income during COVID-related financial hardship:
- W-2's
- Pay stubs
- Tax Returns
- IRS Form 1099
- Written statement from homeowner detailing dates of hardship.
- If applicable, documentation of current income
- Proof of ownership for subject property, such as tax statement from county sheriff
- Most recent bank account(s), all pages.
- Deposit accounts (checking, savings, certificates of deposit, etc.
- Most recent statement(s) for assistance requested
- Mortgage statement
- Past due property tax bill, can be no later than 2017 tax year.
- Past due Homeowners or Flood Insurance bills, if not part of mortgage escrow.
- Past due Homeowners Association Dues bill.
- Past due utility bills (water/sewer, electric, and gas).
The Housing Counselor may ask for additional documentation not included on this list, if needed.
