Welcome! Getting started on your Educational Benefits Form (EBF)

All Jefferson County Public Schools (JCPS) students attending Community Eligibility (CE) Provision schools will receive a breakfast/lunch at no charge, regardless of income or completion of this form. However, to determine eligibility for various additional state and federal program benefits that your child may qualify for and to determine funding for your school, please complete this online EBF form.

  • If all JCPS students in the household attend CE schools, please complete this online Educational Benefits Form or the paper version.
  • If any K-12 JCPS students in the household attend non-CE schools and you wish to apply for Free and Reduced-Price School Meals, please complete a red Application for Free and Reduced-Price School Meals instead. The application is also available online at www.MySchoolApps.com.
  • If any JCPS students in the household attend an Early Childhood program, please complete a red Application for Free and Reduced-Price School Meals so that we can capture eligibility for Snacks instead. The application is also available online at www.MySchoolApps.com.
  • Only one form is needed per household with all students listed on the same form.
  • To find out if your schools are participating in CE, please check our website at www.jcpsfood.org .
  • If you received a letter this school year indicating that your child is directly certified, please read the letter carefully - you may not need to complete a form.
  • Call us at 485-6237 with any questions.
    • What you will need to provide during the online form process:

    • The names and incomes of every member of your household (unless the household receives SNAP, KTAP or FDPIR).
    • Don't forget to include infants, toddlers and students attending non-JCPS schools, grandparents, friends or other relatives who share income and expenses.
    • (Optional) The school, grade, and birthdate of every student in your household.
    • (Optional) A valid email address or phone number for district communication about the status of the form.
