School Nurse Performance Evaluation  Survey 
Hello School Nurses:  My name is Freddi Bastidas, and I am a BSN student at Wenatchee Valley College. Every year, we complete clinical projects that are tailored to improving nursing resources around our area and, ultimately, the entire state. I am working on a project in collaboration with the SNOW Research Committee to collect information about school nursing performance evaluation processes and tools, and your expertise matters! The information collected will be used to support process improvement efforts.   

This survey has up to 26 questions (depending on responses), including demographic questions and questions pertaining to your experiences with school nursing performance evaluations. It will take approx. 10 minutes to complete. All data is confidential and neither you or your school are identifiable. If you work for more than one school district, please complete a separate survey for each district.This survey will be open until Friday, March 8.
