A few things to know for this form:
1. You and at least one parent/guardian need to go through this form TOGETHER!
2. The form will take you 40-50 minutes to complete
3. Have an open mind!  The fun of band is less what instrument you play and more about the music and experiences of being in band.  
4. At the end of this form you will indicate your top brass and woodwind instrument choices
5. Students interested in percussion must select their top woodwind or brass instruments and then will have additional steps to complete to audition for percussion.  More information for percussion is contained in this form.
