2022 Junior Lancer Volleyball Player Registration & Waitlist
Welcome to Junior Lancer Volleyball!

IMPORTANT:
Please note that registration is ONLY open for our 5th grade teams.

Other interested players may be placed on a waitlist. Read on for more information!

Players entering 4th or 5th grade in the fall:
Registration will remain open until three 5th grade teams are full.
Please complete all of the steps to complete your registration and guarantee a spot on a team!
Teams will be formed during the first week of practice in August.

Players entering 6th, 7th, or 8th grade in the fall:
Registration closed on April 30th. If you wish to be placed on the waitlist, please fill out this Google Form.
Do not fill out the printed registration forms or turn in a check unless you hear from us.

Here are the steps to complete registration:
1. Fill out 2022 JLVB Player Registration Google Form between April 1st and April 30th.
2. Print 2022 JLVB Forms Packet and fill out forms:

3. Write a check for $300.00, made out to Junior Lancer Volleyball.
4. Bring complete registration to Forms & Fees night, conveniently held during our Serving Others Tune Up Clinic at Brookfield Central on May 10th and 11th. Board members will be set up in the hall outside the Main Gym to receive registration materials and to answer questions from 5:00-8:00pm both nights.

A complete registration includes:
-Completed 2022 JLVB Player Registration Google Form
-Parent Code of Conduct
-Player Code of Conduct
-Anti-Bullying Code of Conduct
-Medical Release Form
-Concussion Agreement & Information Forms
-Check for $300.00
