ASD/Chugiak High School

To park on campus at Chugiak High School the following must be completed online:

1.  This form MUST be completed by someone 18 years or older.
2.  COMPLETION of this google form.
3.  Once ALL information is completed you will need to visit the Activities Office at Chugiak High School to retrieve your physical parking permit.
