I, the undersigned, individually, and as parent and/or guardian of the player registered above, a minor, ask that she be permitted to participate in this Field Hockey Camp sponsored by the Annville Cleona Field Hockey Staff. I do herby agree to release, discharge and hold harless Annville Cleona Field Hockey Staff, Annville Cleona School District and/or any other program volunteers, from all causes, liablities, damages, claim, or demands whatsover on account of any injury or accident involving the said minor arising out of the minor's attendanc at this activity.
