Parent addendum to the Student Device Agreement
Dear Parent/Guardian,
Please be aware that you and your student are responsible for the following:

Safety: The device your student is being assigned is not a toy.  It is electronic equipment that uses electricity from a battery and a power cord that plugs into a wall outlet.  In addition to the hazards of electricity, batteries and power cords produce heat that could be harmful.  Please be sure you have reviewed the safety precautions in the Student Device Handbook (http://bit.ly/DeviceBk) and that your student always uses these precautions. *
Loss and Accidental Damage: Your student's use of a device will be limited or suspended if his/her device is lost or accidentally damaged.  Your student may have to complete retraining and/or other corrective actions before use is fully restored. *
Inappropriate Use: Your student's use of the device will be limited or suspended for using the device for inappropriate or unapproved purposes.  Your student may have to complete retraining and/or other corrective actions before use is fully restored. *
Intentional Damage: Your student's use of a device will be limited or suspended if s/he intentionally damages it.  Your student will have to complete retraining and/or other corrective actions before use is fully restored. *
