BASD K-12 Student COVID-19 Self-Report Form

Please complete the following information upon notification of a confirmed student case of COVID-19.

You must complete a separate document for each positive case. For example, if you have 3 cases to report, the form below will need to be filled out 3 separate times.

Thank you for your submission. Your student will need to isolate at home for 5 full days. Onset of symptoms is day 0. OR test date is day 0 if no symptoms. They may return to school on day 6-10 and wear a mask.

Please also fill out the attendance sheet for proper recording of the absence.
