I, the undersigned, certify that I am the parent or legal guardian of above-named player (hereafter the “minor child”). In consideration of being allowed to participate in any services or activities offered by Gretna Elite Academy (“GEA”) or any related GEA events and activities including activities organized by Gretna Elite Academy at the Gretna Sports Complex, Leo Royal, Capehart, Papillion Landing or other facilities authorized by GEA to host such activities, I, parent/legal guardian, for myself and my minor child, acknowledge, appreciate, and agree that:1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. Assumption of Risk also includes injury, dismemberment and/or death. While particular rules and personal discipline may reduce this risk, the risk of serious illness, permanent disability and death does exist; and, 2. The risk of injury from the services or activities is significant, including the potential for permanent paralysis and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,3. I KNOWINGLY, VOLUNTARILY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my, and my minor child’s, participation; and,4. I acknowledge that I received and read the materials provided by GEA setting forth the guidelines/rules that must be adhered to by all participants and their parents/guardians/chaperones in connection with activities until further notice. I understand that these guidelines/rules are necessary for the health and safety of all participants in the activities and that such guidelines/rules may change at any time.5. I willingly agree to comply with the stated and customary terms and conditions for participation. I take full responsibility for both mine and my child(ren)'s adherence to all such guidelines/rules including self-screening for COVID-19 in accordance with any then applicable Centers for Disease Control and Prevention (“CDC”) or other federal, state or local guidelines prior to each and every participation in the activities. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of GEA immediately; and,6. I, for myself, my minor child, and on behalf of my and our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS GRETNA ELITE ACADEMY, ITS officers, officials, agents, administration, coaches, volunteers and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the activity ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property associated with my presence or participation, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT , FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. *
