Information from SJCSD dated April 15, 2021

CDC updates regarding quarantine requirements after vaccination:
 
  • Individuals who have been fully vaccinated – that is, 2 weeks or more have passed since the second dose in a 2-dose vaccine or the first dose in a 1-dose vaccine series – will not be required to quarantine following exposure, provided he/she does not develop new symptoms of COVID-19. 
 
The Department of Health (DOH) requests that we follow the current contact tracing protocol, regardless of vaccination status. They will contact exposed individuals to ensure no symptoms have developed, research his/her vaccination status in the state system, and provide further COVID education. The DOH will determine if the individual is exempt from quarantine. The Health Services Department, as well as the school nurses, will work diligently to assist students, parents and staff in this process.

Information from School Messenger dated January 8, 2021

Below are a few important reminders as students and staff have returned to campus.

  • If your student is required to quarantine due to a Covid-19 related matter, or if you are unsure, please contact the clinic 547-7313
  • Please see the attached document for our health screening entry form.
  • Students experiencing any of the symptoms on the sheet attached should remain home.
  • Temperatures will continue to be taken in 1st period each day.
  • If your student is currently awaiting a COVID-19 test result they should remain home until receiving their results.
  • If somebody in the same household as your student has tested positive for COVID-19 they will need to quarantine for 10 days from the last day of contact. Please contact the school nurse.
  • The quarantine period may be shortened to as few as 7 days if a negative PCR test result is obtained on day 6 or later from exposure to the positive case. The Dept. of Health will assist in clearing your child to return to school once they have received the negative test result.

Please do not hesitate to contact the school with any questions.

We appreciate your continued assistance in supporting our health and safety protocols here on campus. As a reminder, below is the student health screener questions. If you can say yes to any of the symptoms below for your child, please keep your child at home until you can say no to all symptoms. If you can say yes to the questions, follow the instructions in red.
 
Student Health Screening Entry Form

Please assess your child daily for the following symptoms and answer the contact questions.

• Fever of 100.4 or higher
• Uncontrolled cough
• Shortness of breath or difficulty breathing
• Sore throat
• Loss of sense of smell or taste
• Muscle aches
• Vomiting or diarrhea

• Is your child currently awaiting COVID-19 test results? If yes, please keep your child home and notify the school nurse when test results are received. Further instructions will be discussed at that time.

• Does your child live in the same household with someone positive for COVID-19? If yes, your child must quarantine for 10 days from the last date of contact with the positive individual. The quarantine period may be shortened to as few as 7 days if a negative PCR test result is obtained on day 6 or later from exposure to the positive case. ATHLETIC BOOSTER CLUB - BOARD MEMBERS NEEDED
The Creekside Knights Athletic Booster Club (CKABC) is a non-profit organization dedicated to supporting the Knights. We are parents, coaches, students, faculty, administration, alumni, and community members dedicated to promoting excellence in interscholastic athletics. We seek to enrich the high school experience and foster success in the classroom for all students by building school spirit and “Knights Pride” through all sports.
 
We have openings and are actively seeking nominees for the 2021-2022 CKABC board. If you are committed to the mission of supporting our athletes, coaches, fans, and staff, please consider serving on the board. The new board is elected in May!

Our @creeksidevolleyball program is GROWING‼️

Beach Volleyball is a Spring Sport at Creekside and you could help make our new court a reality! These courts can be utilized for all athletes for strength and agility training. Athletes in the county will benefit from an additional venue to play sand volleyball.
The cost of one court is $40,000. You may use the link below to help out our newest athletic program at Creekside:

Senior Final Exams will be administered May 18th thru May 21st. The last day for seniors who have met all graduation requirements will be May 21st. However, any seniors who have not met graduation requirements will be expected to continue to attend until all are satisfied.

5/25 - Baccalaureate – 6pm at San Juan Del Rio Church
5/28 - Senior Debt Day - 9am - 11am - see below for specifics

Parents, if you would like a sibling to have left over lunch money transferred to their account when your senior student leaves or your senior is due a refund of remaining lunch money, please email Ms. Marino or send a note to school for your student to give to the Cafe'.

Senior Debt Day is May 28.  

Senior dues must be paid in order participate in Debt Day. Please note specific times for pick-up:

Brick and Mortar students – 9am -12pm
Distance Learner students – 1pm – 3pm

Pick-up for BOTH groups is in front of the auditorium.

Seniors will receive a debt day packet with 10 graduation tickets and any other items needed for graduation.

Attention Seniors!

If you are planning to use Bright Futures to pay for summer classes, please read the information below. 

High school graduates seeking immediate summer funding for postsecondary education should try to gain eligibility during their first evaluation by turning in service hours and auditing all requirements. This will ensure that the student is determined eligible before the start of the semester facilitating a smooth transition of summer financial aid. Students awarded a scholarship later in the summer may be required to pay out of pocket for summer classes and be refunded later in the term. 

There will be no extension of tests scores (test taken through June 30, 2021) or service hours. Service hours must be completed and turned in by the last day of school for seniors which is May 21th. 
CLASS OF 2021 SENIORS - SENDING TRANSCRIPTS 

It is time to start sending your transcripts out to colleges. Before yo do that request a copy of your transcript to review. Things to make sure are accurate on your transcript:

  • correct spelling of name
  • correct address
  • social security number
  • courses taken - including all virtual courses and SJRSC courses
  • correct number of service hours
  • SAT/ACT scores are accurate

To request transcripts, please visit www.parchment.com. Below are directions on how to create your Parchment account to request and send transcripts. Before requesting a transcript, make sure to update it. This way colleges will see the most up to date transcript available. 

If you have questions about your transcript please email your school counselor for assistance.

Returning Library Books and Textbooks
Students if you have library books and textbooks that you are no longer using please bring them to library to be checked in. You must turn in all checked out items before the end of the school year. 

Checking Your Account
Students can check to see what materials are assigned to them by visiting:
 
Click Log In and use your S number and Password
Click on “MY INFO” and this will show all library books, textbooks, and fines that are owed to the library.

LOST OR DAMAGED MATERIALS
If you have lost or damaged a library book or textbook, please see someone in the library to find out about your options.
 
CREEKSIDE KUDOS, CLUBS, & TEAMS
Coach Estela is excited to announce:

Creekside's girls tennis team won the Regionals finals played at home. It was a tough competition, but we beat Flagler Palm Cost and heading to States next week in Orlando!

NEHS will be accepting new membership applications starting April 15th through April 30th.

Students can pick up an application in rooms P20, 210, or from their English teacher.

All applications are due to P20 no later than April 30th.
Accepted students will be notified through their English class the following week.

COUNSELOR CORNER - GUIDANCE

This school year Creekside students will have the opportunity to reach out to colleges and universities virtually. This means Creekside will not host face-to-face visits on campus. Instead, the link below provides student with admission contact information and virtual tour links to view after the school day.


Virtual information will be updated frequently. However, if you are interested in a college/university, please checkout their websites. More than ever, it is easy to find virtual presentations, tours, and admissions contact information.
ATTENDANCE POLICIES

We are almost there Knights. Let's give it our all and arrive on time. PLEASE GIVE YOURSELF PLENTY OF TIME TO MAKE IT TO SCHOOL BEFORE THE 9:20 TARDY BELL RINGS!

(That means allow enough time for car lines and the walk to class after arriving on campus.)
This year, the Absentee Form will be submitted online. Please use the Online Absentee Form link or QR code provided to submit a note when your student in absent. Online absence notes are required to be submitted within 2 days of the student returning back to Creekside in order to be excused.
