HOME STUDENT HEALTH SCREENER QUESTIONS
Parents,

We appreciate your continued assistance in supporting our health and safety protocols here on campus. As a reminder, below is the student health screener questions. If you can say yes to any of the symptoms below for your child, please keep your child at home until you can say no to all symptoms. If you can say yes to the questions, follow the instructions in red.
 
Student Health Screening Entry Form

Please assess your child daily for the following symptoms and answer the contact questions.

  • Fever of 100.4 or higher?
  • Uncontrolled cough?
  • Shortness of breath or difficulty breathing?
  • Sore throat?
  • Loss of sense of smell or taste?
  • Muscle aches?
  • Vomiting or diarrhea?
  • Is your child currently awaiting COVID-19 test results? If yes, please keep your child home and notify the school nurse when test results received. Further instructions will be discussed at that time.

  • Does your child live in the same household with someone positive for COVID-19? If yes, your child must quarantine for 14 days from the last date of contact with the positive individual. A negative test result does not reduce the quarantine requirement for the full 14 day incubation period.

  • Has your child had close contact with someone who in the past 14 days who tested positive for COVID-19? If yes, your child must quarantine for 14 days from the last date of contact with the positive individual. A negative test result does not reduce the quarantine requirement for the full 14 day incubation period.

ATTENDANCE POLICIES
This year, the Absentee Form will be submitted online. Please use the Online Absentee Form link or QR code provided to submit a note when your student in absent. Online absence notes are required to be submitted within 2 days of the student returning back to Creekside in order to be excused.

CLASS OF 2021 SENIORS - SENDING TRANSCIPTS 

It is time to start sending your transcripts out to colleges. Before yo do that request a copy of your transcript to review. Things to make sure are accurate on your transcript:

  • correct spelling of name
  • correct address
  • social security number
  • courses taken - including all virtual courses and SJRSC courses
  • correct number of service hours
  • SAT/ACT scores are accurate

To request transcripts, please visit www.parchment.com. Below are directions on how to create your Parchment account to request and send transcripts. Before requesting a transcript, make sure to update it. This way colleges will see the most up to date transcript available. 

If you have questions about your transcript please email your school counselor for assistance.

We wanted to remind you that the Bright Futures Scholarship application (FFAA) and Free Application for Federal Student Aid (FAFSA) opened on October 1st.

If you are considering applying to college, you should complete both of these applications.

Please see the district financial aid website (https://www.stjohns.k12.fl.us/guidance/scholarships/) for links to these applications and other resources. 





Remember to turn your hours in to Ms. Hemmer in the Guidance Dept. There is also a basket in the front office. The sooner, the better to avoid a data entry bottleneck.
CREEKSIDE KINDNESS
The Access Point program has painted kindness rocks and hid them around the school campus in support of October being anti-bullying month. Our hope is to provide positive messages in a fun way to help provide “pick me ups” and inspire others to do acts of kindness.

We are asking Creekside students and staff to take a photo when they find a rock and post it to our Facebook page, CHS Rocks Kindness. Please use the hashtag #CHSRocksKindness.

Rocks you find are yours to keep!!

ACADEMY T-SHIRT SALE
ATHLETICS
DANCE TEAM FUNDRAISER

The dance team is having a fund raiser for competition next year, we are selling Nothing Bundt Cakes (yum) if you purchase one, they will be delivered here to the school Tuesday before Thanksgiving.

We have an 8-inch cake ($26) that will feed around 10 people and a 10-inch($36) that she says will feed up to 18 if I am not eating.
The flavors are as follows:

  • Lemon
  • White chocolate Raspberry
  • Classic Vanilla
  • Pumpkin
  • Pecan Praline
  • Marble
  • Confetti
  • Carrot

If you put them in the refrigerator until Thanksgiving and take out 3 hours before serving it will be perfect.

RUNNING OF THE KNIGHTS INFORMATION
Due to the Coronavirus crisis, the 2020 Running of the Knights 5K Fundraiser Run/Walk, initially scheduled for May 15th, was postponed.
The Running of the Knights Committee has opted to reschedule the event as a virtual 5K. Registration for the race is open until November 21st, 2020 at CreeksideKnights.com.
100% of the proceeds from the event go to support the Creekside High School community.

Leonard’s Studios will award a $500 scholarship to the Running of the Knights Logo Contest winner! The winning logo will be used to promote the 2021 race. (See logos from previous races above.) ONLY current Creekside students (and 2020 Creekside graduates) are eligible to enter. See attached form for rules.

If you have an idea for the 2021 race logo, we encourage you to enter! Please fill out the registration form, include your artwork, and submit to room 151. Digitized artwork can be emailed to: tina.northcut@stjohns.k12.fl.us. Submission due date is NOVEMBER 20, 2020. Don't miss out!

Students will receive a confirmation email with their appointment date and time. This email will serve as your pass to leave class and must be shown to your teacher before leaving.

Providing teens with the tools to foster mental wellness and breakdown stigma surrounding mental health.

About this Event
A virtual summit being held for all high school students in St. Johns County, Florida on November 14.

The BRAVE Virtual Summit will provide teens the opportunity to learn about how to foster mental wellness and harness tools to encourage self-care. The summit will be a mixture of lively discussion and education.
The summit will be filled with giveaways, raffle items, workshops, empowering information, and more!

Flagler Health+ Care Connect is proud to host our inaugural BRAVE Virtual Summit, in conjunction with our community partners.

We are thrilled to have Ross Szabo as our guest speaker, a dynamic mental health advocate with lived experience.

The link with information is listed below and we are going to be sending out boxes of resources to whoever has registered AND attended the virtual BRAVE summit. There are also great prizes including Ipod earbuds and gift cards.
 
Certificates of attendance that would be a great addition to a college portfolio detailing that the student attended a two hour summit on mental health education and resources.
 
Spring Dual Enrollment at St. Johns River State College
SJRSC spring course catalog is live as of today. Please peruse the course catalog to complete the attached registration form. All registration forms are due no later than noon on November 30, 2020. Things to keep in mind:

  • If you are a current CHS distance learner and wish to take your SJRSC 2nd term classes online via the college please let me know ASAP so we can drop the courses from your CHS schedule. Then return the attached registration form by the deadline date.
  • If you are an early admissions student who attends the college full-time, you must register for a minimum of 12 credit hours and a maximum of 16 credit hours. 
  •  If you are a part-time DE student at the college and still attend classes at Creekside for some of the day, the maximum credit hours you can take are 10 – this is a combination of CHS DE classes and the college DE classes. When choosing classes for the spring term, be mindful of your CHS schedule – we will not change your CHS schedule to accommodate classes at the college.
  • Current DE students only need to turn in a registration form for spring DE classes at the college site.
  • If a student has never taken a DE class and would like to take a spring DE class, they will need to complete an online SJRSC application, submit official SAT, ACT or PERT scores and the registration form all before 11/30.

Virtual information will be updated frequently. However, if you are interested in a college/university, please checkout their websites. More than ever, it is easy to find virtual presentations, tours, and admissions contact information.
EXPLORING COLLEGE OPTIONS
Exploring College Options is a special informational program sponsored by the undergraduate admissions offices of five of the country's leading universities: Duke UniversityGeorgetown UniversityHarvard University, the University of Pennsylvania, and Stanford University.
This fall we are pleased to invite you and your family to join us for a 90-minute webinar to help you explore your college options and prepare a strong and reflective application in this time of change and new challenges. During the program, admission officers from our five universities will introduce their institutions, discuss the college application process, answer general questions, and provide individual school breakout sessions.

Please register for the webinar date and time that best fits your schedule.
  
 

College & Career Night 2020

After careful consideration, St. Johns County School District will not be hosting the Annual District College & Career Night that was scheduled for September 9th 2020 at Pedro Menendez High School. Instead, students are encouraged to register for any of the upcoming virtual NACAC fairs. College representatives may continue to schedule virtual or in-person visits to local high schools. Details are forthcoming from Creekside.

For the virtual NACAC Fairs, more than 600 colleges and universities will be online to talk with high school students on each of these dates this fall:

  • Sunday, Nov. 8 from 2 – 10 p.m. ET

Each virtual fair is a free, one-day event. Attend as often as you like. Parents invited, too.

Register at virtualcollegefairs.org and then log in on the fair dates using your phone or computer.
Create your itinerary by selecting which colleges you’re interested in and attend their live and interactive Zoom sessions. Also available are NACAC Meet-Ups on finding your best college fit, paying for college, and more.

Sessions cover topics such as how to apply, financial aid, student life, and more. Watch, learn, and get your questions answered about academics, athletics, diversity, study abroad, and other topics. You can also meet one-on-one with a college’s admission counselor by choosing a convenient virtual meeting date and time.

Mini Camp College
Given the inability to host programs in person during spring/summer 2020, but holding to our unwavering commitment to demystify the complexities of college admissions and promote access to higher education to first-generation/underrepresented students, the Inclusion, Access, and Success Committee devised a way to deliver this content in a virtual format through a series of five Mini Camp College webinars. These webinars have now concluded. We invite you to share the links below with your students and their families as they navigate their college search and application process.
