HOME STUDENT HEALTH SCREENER QUESTIONS
Parents,

Information from School Messenger dated January 8, 2021

Below are a few important reminders as students and staff have returned to campus.

  • If your student is required to quarantine due to a Covid-19 related matter, or if you are unsure, please contact the clinic 547-7313

  • Please see the attached document for our health screening entry form.

  • Students experiencing any of the symptoms on the sheet attached should remain home.

  • Temperatures will continue to be taken in 1st period each day.

  • If your student is currently awaiting a COVID-19 test result they should remain home until receiving their results.

  • If somebody in the same household as your student has tested positive for COVID-19 they will need to quarantine for 10 days from the last day of contact. Please contact the school nurse.
  • The quarantine period may be shortened to as few as 7 days if a negative PCR test result is obtained on day 6 or later from exposure to the positive case. The Dept. of Health will assist in clearing your child to return to school once they have received the negative test result.

Please do not hesitate to contact the school with any questions.

We appreciate your continued assistance in supporting our health and safety protocols here on campus. As a reminder, below is the student health screener questions. If you can say yes to any of the symptoms below for your child, please keep your child at home until you can say no to all symptoms. If you can say yes to the questions, follow the instructions in red.
 
Student Health Screening Entry Form

Please assess your child daily for the following symptoms and answer the contact questions.

• Fever of 100.4 or higher
• Uncontrolled cough
• Shortness of breath or difficulty breathing
• Sore throat
• Loss of sense of smell or taste
• Muscle aches
• Vomiting or diarrhea

• Is your child currently awaiting COVID-19 test results? If yes, please keep your child home and notify the school nurse when test results are received. Further instructions will be discussed at that time.

• Does your child live in the same household with someone positive for COVID-19? If yes, your child must quarantine for 10 days from the last date of contact with the positive individual. The quarantine period may be shortened to as few as 7 days if a negative PCR test result is obtained on day 6 or later from exposure to the positive case. The Dept. of Health will assist in clearing your child to return to school once they have received the negative test result.

Attached you will find the updated CDC and Florida Department of Health guidelines for Quarantine. There are a couple of items we wanted to highlight:

  1. The testing method of reducing the quarantine does not allow for a rapid antigen test only a PCR test will qualify.
  2. A PCR test does take longer to get the results back therefore the testing method may not be the opportunity to return back to school quicker since the test has to be taken no earlier than on day 6.
  3. If a student is waiting on a COVID test result, the student should be at home until they receive the results.

CHS CAREER ACADEMY APPLICATION INFO
Career Academies at Creekside

Freshmen, did you know that you can apply to be in an Academy for the 2021-2022 school year? This is your last opportunity to apply. Applications are open through students’ HAC accounts from January 8th until February 5th. Students can select up to three academies to apply for on the application, but you must rank them in the order of your preference.

In addition to the Virtual Academy Showcase that is hosted by the District on Tuesday, January 12 starting at 3:30, each high school in St. Johns County is also hosting a Virtual Academy Night for prospective students to obtain information and talk to current students and teachers. For those who are interested, I encourage you to attend one or both of these events. Creekside’s Academy Night is scheduled for Thursday, January 21, starting at 6:00. Both links will be posted on the CHS home page and the CHS Academies home page on their respective dates.

Here at Creekside we have three amazing Academies.

  • In the Academy of Emerging Technology, we have the Digital Media strand and the Cybersecurity strand. 

  • In the Academy of Engineering and Environmental Sciences, we have the Engineering strand and the Environmental Sciences strand.


Don’t miss out, Freshmen, if you want to join an Academy for next school year. If you have any questions, please feel free to stop by and see Mrs. Kelly in the main office.
DISTRICT FINAL EXAMS
District created finals exams for semester courses will be held the week of January 19, 2021.
There will be no change to the daily bell schedule while we administer exams.

Distance Learners will test separately from the Brick and Mortar students. Testing will occur in the auditorium so students will not need to enter campus. Social distancing guidelines will be in place as well as district screening guidelines. Please see the attached schedule & exam locations for more specific information. Teachers will also be communicating details regularly as we approach the testing date.

Congratulations to the Creekside Theatre students on a successful fall season! Pictured from top to bottom are the casts from The Same But Different (an original one-act play that earned a superior rating at the virtual regional theatre festival), Macbeth, The Nutcracker, and The Wonderful Wizard of Oz.

CLASS OF 2021 SENIORS - SENDING TRANSCIPTS 

It is time to start sending your transcripts out to colleges. Before yo do that request a copy of your transcript to review. Things to make sure are accurate on your transcript:

  • correct spelling of name
  • correct address
  • social security number
  • courses taken - including all virtual courses and SJRSC courses
  • correct number of service hours
  • SAT/ACT scores are accurate

To request transcripts, please visit www.parchment.com. Below are directions on how to create your Parchment account to request and send transcripts. Before requesting a transcript, make sure to update it. This way colleges will see the most up to date transcript available. 

If you have questions about your transcript please email your school counselor for assistance. 
SPORTS / CLUB NEWS

Any boys interested in being on the boys weightlifting team that have not already been to pre-season practices, please see Coach Stalvey in the Weight room as soon as possible. Official practice starts this coming Monday.
Tryouts will take place for 3 days beginning on Monday, January 18th. Please arrive dressed in proper attire with your softball equipment. 

1/18: 2:00 - 4:30 pm 
1/19: 4:15 - 6:30 pm
1/20: 4:15 - 6:30 pm 

We are mindful that students are being quarantined on a daily basis. If this is the case for a potential athlete, we will mostly certainly provide a flexible plan for them to tryout during these challenging times.

Choral Music Auditions
February 1 – 5, 2021
 
 
For the first time ever, all Creekside High School students will be able to audition for one of three choral ensembles at Creekside HS (Chorus, Vocal Ensemble and Chamber Choir). The Choral Department has been very successful in Concert Music Performance Assessments as well as participating at Disney’s Candlelight Processional and sang with rock band Foreigner.

See Mr. Hall in Room 490 before 9 am any day to schedule an audition time.
COUNSELOR CORNER - GUIDANCE
Students may EMAIL their school counselor for questions that do not require an appointment.
Reasons you would need an appointment with your counselor:

Students will receive a confirmation email with their appointment date and time. This email will serve as your pass to leave class and must be shown to your teacher before leaving.

Parents, please note, all parent meetings with school counselors are held virtually via Microsoft Teams. Please email your student's school counselor to schedule an appointment.

We wanted to remind you that the Bright Futures Scholarship application (FFAA) and Free Application for Federal Student Aid (FAFSA) opened on October 1st.

If you are considering applying to college, you should complete both of these applications.
