HOME STUDENT HEALTH SCREENER QUESTIONS
Parents,

We appreciate your continued assistance in supporting our health and safety protocols here on campus. As a reminder, below is the student health screener questions. If you can say yes to any of the symptoms below for your child, please keep your child at home until you can say no to all symptoms. If you can say yes to the questions, follow the instructions in red.
 
Student Health Screening Entry Form


Please assess your child daily for the following symptoms and answer the contact questions.

  • Fever of 100.4 or higher?
  • Uncontrolled cough?
  • Shortness of breath or difficulty breathing?
  • Sore throat?
  • Loss of sense of smell or taste?
  • Muscle aches?
  • Vomiting or diarrhea?
  • Is your child currently awaiting COVID-19 test results? If yes, please keep your child home and notify the school nurse when test results received. Further instructions will be discussed at that time.

  • Does your child live in the same household with someone positive for COVID-19? If yes, your child must quarantine for 14 days from the last date of contact with the positive individual. A negative test result does not reduce the quarantine requirement for the full 14 day incubation period.

  • Has your child had close contact with someone who in the past 14 days who tested positive for COVID-19? If yes, your child must quarantine for 14 days from the last date of contact with the positive individual. A negative test result does not reduce the quarantine requirement for the full 14 day incubation period.

Creekside PTSO is having a drawing for 1 premium parking space for the 2020 - 2021 school year.

Juniors and Seniors Only - Creekside PTSO is having a drawing - $10 per ticket - for 1 premium parking space for the 2020 - 2021 school year. All tickets will be put in a computer generated system to select one winner, the more tickets you buy the more chances you have to win! The parking space is located in the front of the student lot - ALL YEAR LONG. No more "hoping" for a spot and no more "fighting" for a spot, you can drive in at any time in the morning at your leisure and pull right in. No hassle, no stress!

Please ask your student to turn their completed/signed form in to their 1st period teacher this week. Distance Learners may send their completed/signed forms via email to Ms. Mackoul (click this link).

ATTENDANCE POLICIES
A REMINDER TO PARENTS with students on campus:

During school hours if your child is SICK, please have them go to the nurse and the nurse will call you to pick them up. We encourage this procedure instead of the student calling you from their cell phone to check them out.
Also, please go to the Creekside website link below to find the appropriate forms to have your child checked out from school for appointments off campus during the school day. The student will bring in the Early Dismissal form first thing in the morning on the day they are to be checked out early and they will receive a pass to get out of class or to drive if they are a driver.  It is the responsibility of the front office staff to minimize class disruptions during instruction time.

This year, the Absentee Form will be submitted online. Please use the Online Absentee Form link or QR code provided below to submit a note when your student in absent. Online absence notes are required to be submitted within 2 days of the student returning back to Creekside in order to be excused. CONGRATULATIONS ELECTED STUDENT GOVERNMENT OFFICERS
Creekside High School would like to congratulate Ali Pressel for being selected as one of only eight National Geographic’s Emerging Explorers from around the world this year. She is only the second educator to receive this honor in the program’s 18 years of existence. We are very fortunate to have Ali as a faculty member and Academy teacher here at Creekside.
Thank you everyone for a job well done! You all deserve to be celebrated.

It's yearbook ad time! Creekside's Yearbook staff is already working hard on the yearbook, and we want you to be part of it! Senior Parents: The deadline for ordering an ad is October 22nd.

To create and purchase an ad for your senior, log in to yearbookordercenter.com using our school code (10495). Click "Buy a Yearbook Ad" to start your purchase and design. Use the links below "Buy a Yearbook Ad" to help you design the perfect ad for your student. PTSO MEMBERSHIPS
The SJCSD has been hard at work creating a flexible, responsible, and comprehensive plan for returning to school for the 2020-2021 school year. The main focus is on safety and high-quality education for all students. With that in mind, the PTSO membership drive will be completely online. We need your support as we move through this monumental school year.

Here's a quick update from the Girls Golf Team. We had two matches this week
 
The first match was a 3 team event vs. Pedro Menendez & St. Augustine 
Creekside was victorious vs. both teams. 
  • Creekside team total 193 
  • Pedro Menendez total 228
  • St. Augustine withdrew during match 
  • Low gross leaders were: Paisley Bratton Sr 42, Katie Andersen Jr 43 & Ava Anderson Jr 52 
 
2nd Match vs. Nease High School
  • Creekside team total 200
  • Nease team total 161

With your subscription, the NFHS Live Stream Network grants you live stream access to competitions across the nation. The value of this subscription alone is $69.99 and the offer expires 12/31.

With limited seating capacity (below 30%), our venues will likely sell out. Your subscription will allow you to livestream all home Creekside Stadium and Gymnasium games (football, soccer, lacrosse, track, volleyball, basketball, and wrestling) as well as away games at Ponte Vedra High School, Bartram Trail High School, Nease High School, Pedro Menendez High School, Bolles, Episcopal, Bishop Kenny, and hundreds of other schools.

Link Crew Boo Grams

It's Boo gram time! Link Crew is putting together Boo grams for organizations that serve the less fortunate along with our wonderful staff. We need donations of bags of individually wrapped candy. Donations can be dropped off at parent pickup to Dean Bundshuh, Dean Bundshuh’s office, or Ms. Alder's room 625. CLASS OF 2021 SENIORS - SENDING TRANSCIPTS 

It is time to start sending your transcripts out to colleges. Before yo do that request a copy of your transcript to review. Things to make sure are accurate on your transcript:

  • correct spelling of name
  • correct address
  • social security number
  • courses taken - including all virtual courses and SJRSC courses
  • correct number of service hours
  • SAT/ACT scores are accurate

To request transcripts, please visit www.parchment.com. Below are directions on how to create your Parchment account to request and send transcripts. Before requesting a transcript, make sure to update it. This way colleges will see the most up to date transcript available. 

If you have questions about your transcript please email your school counselor for assistance. 
Interested in applying to a State University in Florida for the summer or fall of 2021?

This school year Creekside students will have the opportunity to reach out to colleges and universities virtually. This means Creekside will not host face-to-face visits on campus. Instead, the link below provides student with admission contact information and virtual tour links to view after the school day.

EXPLORING COLLEGE OPTIONS
Exploring College Options is a special informational program sponsored by the undergraduate admissions offices of five of the country's leading universities: Duke UniversityGeorgetown UniversityHarvard University, the University of Pennsylvania, and Stanford University.
This fall we are pleased to invite you and your family to join us for a 90-minute webinar to help you explore your college options and prepare a strong and reflective application in this time of change and new challenges. During the program, admission officers from our five universities will introduce their institutions, discuss the college application process, answer general questions, and provide individual school breakout sessions.

College & Career Night 2020

After careful consideration, St. Johns County School District will not be hosting the Annual District College & Career Night that was scheduled for September 9th 2020 at Pedro Menendez High School. Instead, students are encouraged to register for any of the upcoming virtual NACAC fairs. College representatives may continue to schedule virtual or in-person visits to local high schools. Details are forthcoming from Creekside.

For the virtual NACAC Fairs, more than 600 colleges and universities will be online to talk with high school students on each of these dates this fall:

  • Monday, Oct. 12 from 1 – 9 p.m. ET
  • Sunday, Oct. 18 from 12 – 8 p.m. ET
  • Sunday, Nov. 8 from 2 – 10 p.m. ET

Each virtual fair is a free, one-day event. Attend as often as you like. Parents invited, too.

Register at virtualcollegefairs.org and then log in on the fair dates using your phone or computer.
Create your itinerary by selecting which colleges you’re interested in and attend their live and interactive Zoom sessions. Also available are NACAC Meet-Ups on finding your best college fit, paying for college, and more.

Sessions cover topics such as how to apply, financial aid, student life, and more. Watch, learn, and get your questions answered about academics, athletics, diversity, study abroad, and other topics. You can also meet one-on-one with a college’s admission counselor by choosing a convenient virtual meeting date and time.

Mini Camp College
Given the inability to host programs in person during spring/summer 2020, but holding to our unwavering commitment to demystify the complexities of college admissions and promote access to higher education to first-generation/underrepresented students, the Inclusion, Access, and Success Committee devised a way to deliver this content in a virtual format through a series of five Mini Camp College webinars. These webinars have now concluded. We invite you to share the links below with your students and their families as they navigate their college search and application process.
