Information from SJCSD dated April 15, 2021

CDC updates regarding quarantine requirements after vaccination:
 
  • Individuals who have been fully vaccinated – that is, 2 weeks or more have passed since the second dose in a 2-dose vaccine or the first dose in a 1-dose vaccine series – will not be required to quarantine following exposure, provided he/she does not develop new symptoms of COVID-19. 
 
The Department of Health (DOH) requests that we follow the current contact tracing protocol, regardless of vaccination status. They will contact exposed individuals to ensure no symptoms have developed, research his/her vaccination status in the state system, and provide further COVID education. The DOH will determine if the individual is exempt from quarantine. The Health Services Department, as well as the school nurses, will work diligently to assist students, parents and staff in this process.

END OF YEAR TESTING INFORMATION - DISMISSAL INFO
Dear KNIGHTS,

As the school year comes to an end and the testing season begins, it is our goal to ensure an efficient and positive learning environment. In order to accomplish this, we would like to share some information with you. 

1. The semester exam schedule for the week of June 7-June 10 is posted on the Creekside website. Students will remain in class for the entire scheduled exam block each day, and exams will only be administered during the designated periods and times listed on this schedule.

2. Creekside will follow normal attendance procedures. To ensure a safe and smooth process for all students, we ask you to review and adhere the following check-out procedures on the Creekside website: http://www-chs.stjohns.k12.fl.us/parents/attendance/. As a reminder, we are not able to accept parent phone calls for early check-outs. Early dismissal notes need to be turned into the front office before school begins.

Finally, we appreciate your continued support as we work through exam week. We wish you and your family a safe and relaxing summer.

* Attendance Question of the Week*

During Exam Week, does my student need a note to leave after the exam time if they are not going to lunch? 
Answer: For Monday (6/7), Tuesday (6/8), or Thursday (6/10) – NO, a note is not needed on those days; students may leave after the exams. However, on Wednesday (6/9), there is class after lunch so for that day only, yes, a note will need to be turned into the front office if the student is leaving earlier than the end of the school day.
CREEKSIDE SENIORS - CLASS of 2021
GRADUATION INFORMATION
        
June 4 – Graduation Rehearsal– 10:00am Creekside Football Field
  • Students should arrive no later than 9:45am.
  • All seniors must attend if participating in the Graduation Commencement Ceremony.

June 5 – Graduation – 9:00am (TIAA Bank Field)
  • **Graduation Tickets: Each student will receive 10 tickets. If you need more than 10 tickets, please talk with other seniors to see if any other family has some available to give you.
  • Seniors must be in the staging area at the side of the stadium by 7:45am.
  • We encourage cell phones or any electronics, purses, bags to be left at home or in your car. We are unable able to secure these items.
  • The ceremony starts at 9:00am and will be approximately 2 hours.
  • Every guest attending the graduation must have a ticket. This includes infants in laps. Seating is first come first serve. Disability seating is available. If you or a guest are hearing impaired and require an interpreter, please contact Brooke Mackoul (brooke.mackoul@stjohns.k12.fl.us)
  • Students received a photo card in their debt day packet, they will need to bring this with them on graduation day to have their graduation picture taken by Leonard’s.
  • In your folder each family received one parking pass for graduation. Additional parking will be available, see the attachment for more information.
  • Graduation will be livestreamed. Guests who are unable to attend the in-person ceremony can visit the link below to watch the ceremony. Ceremony will be live during the scheduled event time only. https://tiaabankfield.com/2021-graduation-creekside-high-school 
  • You will receive your diploma packet after the ceremony in the staging area.
  • Any decorations to caps must be flat (2D) and appropriate and not distracting to the ceremony.  Graduates can dress casually but need to be appropriately dressed.
  • Per TIAA stadium, stiletto style shoes are not allowed. Wedges or shoes with a large platform heel are allowed.
IMPORTANT INFO: Taking care of this in advance will make Debt Day much smoother for your Senior. Please read.

Parents, if you would like a sibling to have left over lunch money transferred to their account when your senior student leaves or your senior is due a refund of remaining lunch money, please email Ms. Marino or send a note to school for your student to give to the Cafe'. CLASS OF 2021 SENIORS - SENDING TRANSCRIPTS 

It is time to start sending your transcripts out to colleges. Before yo do that request a copy of your transcript to review. Things to make sure are accurate on your transcript:

  • correct spelling of name
  • correct address
  • social security number
  • courses taken - including all virtual courses and SJRSC courses
  • correct number of service hours
  • SAT/ACT scores are accurate

To request transcripts, please visit www.parchment.com. Below are directions on how to create your Parchment account to request and send transcripts. Before requesting a transcript, make sure to update it. This way colleges will see the most up to date transcript available. 

If you have questions about your transcript please email your school counselor for assistance.

Returning Library Books and Textbooks
Students if you have library books and textbooks that you are no longer using please bring them to library to be checked in. You must turn in all checked out items before the end of the school year. 

Checking Your Account
Students can check to see what materials are assigned to them by visiting:
 
Click Log In and use your S number and Password
Click on “MY INFO” and this will show all library books, textbooks, and fines that are owed to the library.

LOST OR DAMAGED MATERIALS
If you have lost or damaged a library book or textbook, please see someone in the library to find out about your options.

Last weekend we held the second Miss Creekside pageant of the school year with 11 committed and talented contestants. Please join us in congratulating all of the contestants on their hard work and dedication to this process. We are proud to announce that our Miss Creekside 2022 is Britta Strevig. Kayla Vasko was named 1st Runner Up and Davey Ciric was named 2nd Runner Up. Sierra Phillips was named Miss Congeniality by her fellow contestants. Mia Marcia was selected as Most Photogenic. Additionally, Britta Strevig received the most talented award, Kayla Vasko received the Most Scholarly Award, and Ava Anderson received the award for Most Ads Sold.

ATHLETICS - BOOSTER INFORMATION
Our @creeksidevolleyball program is GROWING‼️

Beach Volleyball is a Spring Sport at Creekside and you could help make our new court a reality! These courts can be utilized for all athletes for strength and agility training. Athletes in the county will benefit from an additional venue to play sand volleyball.

COUNSELOR CORNER - GUIDANCE
STUDENTS INTERESTED IN TAKING DUAL ENROLLMENT CLASSES AT ST. JOHNS RIVER STATE COLLEGE FOR THE FALL 2021 TERM - this includes online classes.

The fall course catalog will be released sometime in early June, around June 14th. Students will need to check Schoology daily after the school year ends for the release date and deadline to submit fall registration forms. Once we know the actual release of the fall course catalog, the deadline date will be set. However, keep in mind the deadline date will be sometime before June 30th - so a quick turnaround from the time the fall course catalog is released. 

  • TENTATIVE COURSE CATALOG RELAEASE DATE: JUNE 14TH
  • TENTATIVE DEADLINE TO SUBMIT FALL PAPERWORK FOR CLASSES AT THE COLLEGE SITE: JUNE 30TH

ATTENDANCE POLICIES

We are almost there Knights. Let's give it our all and arrive on time. PLEASE GIVE YOURSELF PLENTY OF TIME TO MAKE IT TO SCHOOL BEFORE THE 9:20 TARDY BELL RINGS!

(That means allow enough time for car lines and the walk to class after arriving on campus.)
This year, the Absentee Form will be submitted online. Please use the Online Absentee Form link or QR code provided to submit a note when your student in absent. Online absence notes are required to be submitted within 2 days of the student returning back to Creekside in order to be excused.
