STATEMENT OF ACCOMMODATIONS POLICY
The Texas A&M Engineering Extension Service (TEEX) will provide equal opportunity to all employees, training participants, applicants for employment and admission, volunteers and the public who have disabilities. The Americans with Disabilities Act (ADA) and Section 504 of the Rehabilitation Act of 1973 require reasonable accommodation and prohibit discrimination with regards to individuals with disabilities.
TEEX Strategic and Education Services (SES) coordinates accommodations and support services for qualified individuals with disabilities to ensure equal opportunities and access to TEEX training and facilities.
WHO SHOULD USE THIS FORM?
Individuals who wish to receive assistance gaining disability accommodations. The form allows individuals to describe their disability(ies) and their accommodations need, as well as information about barriers faced and previous accommodations received.
This completed form should be submitted with official disability documentation. The documentation must provide information on the impact of the disability so that appropriate accommodations can be identified. Documentation may include assessments, reports, and/or letters from qualified evaluators, professionals, or institutions. Common sources of documentation are health care providers, psychologists, diagnosticians, and/or information form a previous school (e.g. accommodation agreements/letters, 504, IEP or ARD documents).
WHAT HAPPENS ONCE THIS FORM IS COMPLETED?
The Access Coordinator will review submitted materials and meet with the individual to discuss reasonable accommodation(s) and support. Appropriate accommodations are determined by SES and the participant based on the documented needs, previous accommodations, and functional limitations of the participant.
A participant will receive a Letter of Accommodation (LOA) listing approved accommodations. The participant is then responsible for delivering the letter of accommodation issues by SES to the instructors and discussing their needs with the instructors.
