Legal Name Change Clinic

Filling paperwork for a name change can often feel overwhelming. The LGBTRC, in collaboration with the Volunteer Lawyers Project, will offer a name change clinic where you can work with a law student to get this paperwork done and filed on your behalf. There are limited spots available for the clinic and given on a first-come-first-served basis. 
 

To participate in the Name Change Clinic you will need to complete the following steps:

1. Fill out this intake form: https://forms.gle/53JJB8q9xvsz7uSF6
2. Upload a high-resolution picture or scanned copy of your certified birth certificate at the end of the intake form.
3. You will be contacted by Cortney Johnson to set up an appointment.

Appointments must be selected and the intake form must be filled out by March 7th.
