COVID-19 Vaccine Information

If you are 18 or older, you can call our COVID Vaccine scheduling line at 724.632.6801, option 9 to get scheduled!
Everyone needs to meet the following guidelines to be COVID-19 vaccinated:
1. If you have tested positive for COVID-19 within the last 90 days and have received treatement with the Monoclonal Antibodies, we will not be able to administer the vaccine to you per the CDC recommendations until 90 days after your treatment date. If you have not received treatment with Monoclonal Antibodies but have tested positive for COVID-19 within 90 days, you will need to be symptom free to have the vaccine administered.
2. If you have had a past severe allergic reaction to a medication, injectable medication or any other vaccine, please contact your Primary Care Physician to obtain a written clearance that you are able receive the Moderna vaccine. You will need to submit this before we can administer the vaccine to you.
3. You must be 18 years or older to receive the COVID-19 vaccine.

Which vaccine is Centerville Clinics administering?
We are currently administering the Moderna Vaccine. This is a two-step immunization which requires the second dose 28 days from the date of the first dose.
